I am an out-of-network provider for all other insurance companies. You will be provided a monthly statement of services for you to submit directly to your insurance company to receive reimbursement. The amount covered by your insurance company will vary depending on your plan.

The benefit of working with an out-of-network therapist is that your records are entirely confidential and will not be entered into an insurance company database.

How to check your benefits

To avoid financial surprises, you are encouraged to contact your insurance provider and ask the following questions:

  • Do I have mental health benefits?

  • What is my deductible, and has it been met?

  • What is my copay or coinsurance for mental health sessions?

  • How many sessions does my plan cover per year?

  • Do I need a referral from my primary care physician?

CANCELLATION POLICY

Please notify your therapist within 24 hours of your appointment if you are unable to keep your scheduled therapy appointment. If you do not notify us within 24 hours of your missed appointment, you will be charged $75.00.

GOOD FAITH ESTIMATE NOTICE

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers are required to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including mental health services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including mental health services. You can ask for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

  • In the event of a subpoena or involvement in legal proceedings, a separate fee schedule will apply for time spent on preparation, court appearances, and travel. These fees are not covered by insurance and are the responsibility of the client.
